I'm SO glad you asked.
People threw me two showers when I was pregnant, and I got SOO many stuffed animals, and so many toys.
Useless!
Diapers Are useful! Books are great; promotes something other than tv. And clothes. But not newborn. Think one season ahead. Everyone buys newborn stuff.

If you live near her, you can give her a "gift certificate" for xx number of home-cooked meals (by you) delivered to her door.
When I had just given birth and was breastfeeding, food was essential in order to nurse, but my husband and I were worn out. Visitors are okay, but most of the time they're more work than help. We used to wish aloud during those first weeks that someone would just come to our door and leave dinner for us. You can only do take-out so much and it's not healthy for nursing moms.
You could also present your cleaning services too in the the same way.
If you knit or crochet, you could make a baby blanket in the colors of the baby's room.
Give baby some books that are favorites from your old childhood.
Clothes are very important. But size is too. The mom will get plenty of newborn, 0-3, 3-6 month clothes. Beyond that, moms get less and less. 1-year and 18-month clothes are a huge and thoughtful gift. By that time, money begins to get tighter, just as baby grows out of all those cute little teeny clothes.
Oompa Toys is my favorite site for high-quality, award-winning educational toys. The special thing about their inventory is that the toys are made by European toy makers, they use birch wood instead of plastic (more sanitary) and best of all...no batteries! It's where I buy my son's toys and toys for nephews and nieces. They ship quickly.
I think the most important thing is function. If you can pair that with originality, you're doing an amazing thing. It's so painful to get stuff that's pretty and useless. Little known among women who have never had a kid is this: pregnant moms typically get very, very little off their registry. So many mom friends I have experienced the same thing. At some point, we had to go out and buy it all.

Do you have a build -a- bear around you? Ask to have the baby's heartbeat recorded by someone at the doctor and take it to build a bear and get the sound of the babys heartbeat put into a teddy bear. A friend of mine did this and I'm planning on doing it for my best friend next month.

I would get some baby books, early reader books, etc... I recommend Boynton's Greatest Hits: Volume 1 and Boynton's Greatest Hits: Volume 2 by Sandra Boynton. Each set costs around $15.
Or I would recommend getting something to put baby pictures in. For a unique very nice photo items I recommend looking around on Exposures (http://www.exposuresonline.com/jump.jsp?itemID=123&itemType=CATEGORY&path=1,2,8,123&viewAll=true)

I have no idea how much it costs or if you have it in your area, but you could try getting a gift certificate for a cleaning service for a few days after the baby is born. If you have to, get some friends of hers to volunteer and present her with a certificate. Or you could get some friends and coworkers together to present her with a booklet of certificates redeemable for cleaning, errands, meals, babysitting for a few hours, etc. that she can turn in when she needs the help.

I always buy a small laundry basket (this can be used for the babies clothes) and fill it with things for the baby such as a pack of diapers, fingernail clippers, a bottle brush, bibs, and shampoos, and lotions. Really you can just use your own imagination and put all kinds of things in there.

Get her a basket of lotions and bath stuff for MOM not the baby. Once that baby is born most people forget mom exists so here is something that will make her feel better about herself.

A nice basket of lotions and pampering things for the mommy to be.
Not to be forgetful of baby but mom always needs these items and people tend to forget her. Don't worry the baby has more onesies and little booties than they have time to outgrow them.
