im doing massage therapy and need my clients to sign that they are happy to recieve treatment without first getting approval from their doctors for any long term medical conditions that are pre-existing for eg asthma. but i dont know how to phrase it. any help would be most apprecitated. thanks

I give my full consent for all massage therapy received by myself and understand that it is solely my responsibility to seek medical advice from my doctor with regards to the possible undesirable effects of therapeutic massage on any pre-existing condition that I may have and therefore (insert company/soletrader) will not be liable for any injury or ailment incurred through neglecting or not obtaining my doctor's advice prior to any massage therapy received.

You can find a few well worded templates online to start out with. A good resource is http://www.lawdepot.com Here you will find a fill in the blank Release Waiver Form. It will be fairly generic but can be tailored to suit your needs pretty quickly. If you still have trouble, the best solution is to contat a business lawyer and have them create one for you. It will probably cost between $500 and $1000 depending on the size of the firm the lawyer is with, but it is money well spent.

Carefully, but seriously try and get hold of a professional bodies protocols and procedures etc. I am an alternative therapist working alongside the NHS with clients with Substance Addictions. If you do not do an extremely professional document the Doc etc who do not embrace your work can and will rip shreds out of you. It has taken me 15 years to establish myself in the English Lake District in the early days they treat you like a cowboy without the stirrups. I wish you all the luck in the world god bless Dirky

Although I am not a massage therapist, I can tell you what my partner did when we started our salon... She got a few "generic" releases from the school she attended. She took these and had a form based on what she needed drawn up by a local lawyer who had just started his practice. They traded services for this and she now feels safer using a form she understands. That's my best advice.
