Glad that you decided to do it.I can share something from experience and books.
First, be aware of all that you do.
One basic ingredient is good, solid self-esteem and complete self-acceptance for yourself as you are now, with all your weaknesses.Repeat to yourself time to time:"I am a splendid, magnificent being.Loving and lovable.Capable of achieving whatever I want.I succeed in everything I attempt"This is called self-talk and you can learn more about it from books listed below.

Transforming our personality is like tuning a string on a guitar or violin.If it is too loose, no sound will come.If you tighten it too much , it will break.

As you let go of undesirable habits, you will need a substitute for your mind to grab hold of.For it will never stay in a vaccum.Try exercise such as gym, jogging, tennis,martial arts....But dont overload.
Keep the company of good books.
Stick simple motivational posters in you room.
Avod people and places that may cause relapse.
In case any relapse occurs(it does to most of us), never beat yourself up.Accept yourself inspite of failure. Withut failing w cannot succeed.Just like the child learning to walk, falls down numerous times.
Do a meditation practice like breath observation or breath counting everyday.This will give calmness so your freedom of choice will increase.
Write down what all you want to achieve -physical, financial,social,family,career.. and set dead lines for each goal.
Break them down into goals for each year, month, week, day.

In your transformation, choose just one good habit to practice for one week.Try to focus on that alone.Leave all others to natural course.Focussing on just one thing at a time gives strength.

Positive re-enforcement is the best way. Reward the new habit you want to see and shun the old bad habit. Works for almost everything like childcare, pet training, and overcoming addictions.
