The third stage of the Saturn V was used to inject Apollo from earth orbit into the lunar transfer module. The docking of the CM with the LM took place just afterwards. There is a nice picture here:

If that is too much detail, the short answer is that the CM separated from the Saturn-V 3rd stage, turned around, moved back toward the 3rd stage, docked with the LM that was stowed in the top of the 3rd stage and pulled it out.
