He considered that to be an experiment, but yes he took LSD 25, he wrote about it in his autobiography.

Experiment is perhaps a misleading word; to most people it signifies patronization and objectivity. For my part I anxiously awaited their personal benefits that could be derived from the experiences, and was quite willing to be less than objective...."

Yes, according to a documentary I saw on Turner Classic Movies. It was given to him by doctors, I think. At that time people didn't think that it was dangerous, apparently.
