-They used a 3 man rotation back then.
-Teams didn't have middle relievers, starting pitchers were expected to pitch complete games. Because of this, pitchers paced themselves much more than they do today, meaning they wouldn't tire as quickly.
-Power hitting was almost non-existant. You didn't have to worry about a slugger busting the game open, for the most part, the worst that could happen would be giving up a single. That meant less stress, and less fatigue, since you weren't pitching for strikeouts.
-The bottom 3-5 guys in a lineup were almost always completetly useless at the plate. You could easily take an inning off, just throw floaters and watch them ground out.

At the time Cy Young was pitching (the turn of the century) the great majority of pitchers completed almost all their games. They also pitched a higher percentage of their team's games. (But how?--this might not answer your question.)

Pitchers back then pitched every other day or so. And they did not have the weird mechanics or fastball speeds that pitchers do today, so their arms lasted longer.
Bats were thicker & heavier back then so the swings weren't as fast & pitches looked fast to the hitter even though the ball was actually traveling slower than it does today.

They didn't have middle relievers and all the relief pitchers like they do now.
Some guys only throw a couple pitches and leave the game.
They teach them in college now train them to be middle relievers and such!

he didn't throw hard. I used to pitch batting practice and throw at least 200 pitches easy because i was just tossing nice and easy. Back then also guys would pitch more often, now its every 5th game. Then it was pretty much whenever, back to back games, every other game, or something like that.
