Yeah, it was Hendrickson against the Expos in 2003. He hit it off Sun-Woo Kim on June 21st of that year:

As a current Blue Jay or former? Chris Carpenter is a former Jay who I'm sure has hit a homer for the Cardinals. If you are talking about while playing for the Jays, my guess will be Halliday? How about Chacin? WIld guesses here
