When filling out forms for my son, in the space marked "father", I just write deceased; but for his passport or something legal like that, I put his full name, then I showed them the certificate of death. I haven't had to deal with any other formats as of yet.

Depends, but it might be best to only list the one, whose still living. Leve the other one, unlisted. Or put late, before the deceased one's name

Mrs. John Jones or Mrs. Mary Jones or Mr. John Jones.
Whatever you think the decesed husbands wife would prefer. My friends mother still has her phone nmber under her husbands name. He died in 1983.
