ihave a fan light switch with toggle for light and slider for fan wires how does this wire up?

switch has red black white green there are 2 14/2 wires from switch to fixture and 1 14/2 feed in switch box

The switch doesn't seem like it has enough wires to independently control the light and fan. Have a model number?

Did you remove some other switch? How were/are the 3 14/2 cables connected up? Hard to know which 14/2 to fixture is for the light and which is for the fan (strange to me it uses two 14/2's). Are you sure both 14/2's go to the fixture?
