Ukraine - Wikitravel
http://wikitravel.org/en/Ukraine

"No visas are required for citizens of the European Union, USA, Canada, Japan, EU, Switzerland, Liechtenstein, Andorra, Vatican, Monaco, Iceland, Norway, San Marino, Romania, Mongolia, Serbia and Montenegro and the countries of the Commonwealth of Independent States (except Turkmenistan). Note that technically this is only to travel to Ukraine as a 'visitor' - studying or working purposes, even if for less than 90 days, should still require a visa to be issued even for the aforementioned countries."

Go to their Embassy. They'll give you something to fill out.
