Whatever the woman's legal name is at the time of death is what will go on a death certificate.
I am a licensed funeral director in the state of Arizona, and it's always the legal name. We sometimes have women who were widowed, remarried, and then pass away. If they kept the last name of the first husband, that would go on the D.C. (death certificate) I guess what I am saying is whatever that woman used on her legal documents I.E. Driver's license, Mortgage docs, Social Security docs, etc. that's what would go on the D.C.
