You will need to eliminate the mole's food supply and you do that by treating your lawn with a turf insecticide like Talstar or Acephate.

My parents had moles once, I think, and the bait looked like those orange peanut-shaped candies. Can't tell you if that worked or not.

To get rid of the moles you need to get rid of the grubs that the moles are eating. There are lawn treatment products to get rid of grubs and, when the grubs are gone, the moles will go elsewhere looking for their dinner.
