Yes, sometimes the tissue tears. Doctors sometimes perform an episiotomy, in which they cut the tissue below the vagina (the perineum), to enlarge the vagina for delivery. Episiotomy are being performed less frequently because it causes more problems that it fixes. An episiotomy actually INCREASES the likelihood that a woman will tear, because the "tear" has already been started. If an infection begins, it is often fatal. Some doctors still insist on performing episiotomies as a matter of course, despite evidence that they are usually unnecessary and harmful.

I know, this is way more info that you wanted. For others who are curious, check the website below.

If a woman has a baby under the doctors care, and he determines that a cut or two of the vagina is called for he will may a small incision at the base of the vagina, to allow the babies head to pass thru easier and less pain. It is called a episiotomy. He will then stitch up and in a few day it is healed. At the time of the episiotomy the woman will not feel anything of that incision.

If you have a baby without a doctor then tearing of the vagina is very possible and healing could be a problem.

This all depends on the woman and the expansion size of her vagina opening and the size of the baby when it is born.

Yes it can happen but nowadays the doctor usually cut her so she doesn't rip and then puts in stitches.

Sometimes the woman naturally tears or the doctor will make an incision so that the woman does not tear because tears are harder and more painful to heal.

Yes, unfortunately. And, they usually cut you rather than allow you to tear. It can lead to rectal problems later such as nerve and muscle dysfunction. Babies are life changing in many ways.
