I can only imagine that it means that two guys are involved in an intimate act of close spacial distance, and may include touching or rubbing each others body parts. And each of them feels some degree of sexual arousal or excitement.
I am sure that you know what it means, but trying to deny the obvious. Let us try to ponder it from another angle.

What isn't natural is for two guys to engage in that kind of sexual activity. However, if they're just youngsters, then it should be just a passing phase, done out of curiosity.
Yes, the one is doing it for the others pleasure, but, sometimes, the one jerking off the other guy is doing it for his pleasure as well. From there, it's just a stone's throw to giving the other guy oral sex, which is generally where this kind of activity goes--and more! That's definitely abnormal--period!
