I see nothing wrong with paying the bill and including the tip on a credit card. Either way, the server gets some kind of tip regardless. And I strongly disagree that we should leave a bigger tip just because we are "short on cash" and need to leave the tip on a card. I try to hardly pay with cash for anything and paying with credit card does not mean I am short on cash, it's just I prefer not to pay w/cash unless I really have to....reserved for cash-only places. I pay my credit card balance every month, so using a credit card is advantageous to me.

I usually pay the bill with my credit card. If I decide to leave a cash tip, I write "cash" in the tip line and then fill in the total amount on the credit card slip. This prevents any confusion or someone filling in an amount. It always pays to be safe!

I use to work as a server and that didn't make a difference. All that mattered was we received a tip. At the end of the shift we would turn our bank in and they would reduce the money we owed back to the restaurant after they subtracted the credit card tips.
however, the only bad thing about getting only credit card tips for servers was that we couldn't lie about how much we made in tips. This made our income go up and therefore we had to pay more taxes.

I almost always use a credit card to pay the bill and pay the tip in cash.

Yes, I do that often. As a former waitress, I think it is more polite if the guests leave a cash tip when possible. However, it is not impolite to leave a tip on a credit card, especially if you will be more generous because you are short on cash in your wallet.

Most definately! I usually try to pay with cash, that way I don't have to wait for the server to come back with my receipt to sign. But when I do pay with my card, I usually pay the tip in cash.

I have often done that, as it somehow makes me feel the server will definitely get the cash.
If the tip is placed on the credit card, I feel unsure as to if the server will be given the tip (just my personal opinion)

Sure. I do it if I have the cash. Make sure that your credit card number isn't written out entirely on your receipt. If it is then black out all of it except for the last four.
