Let the doc know that the Tylenol/ibuprofen is not working.
Heat and ice can help. Moist heat is best.
She may also try laying on her back, and draw her knees to her chest.
Laying flat with pillows under her knees can take a lot of pressure off the lower back.

Back pain can be a terrible thing. What I would do is the following:
Continue the ibuprofen (Advil IS ibuprofen, btw). If she has no kidney or bleeding problems, she may take up to 2400 milligrams (12 tablets) per 24 hours. That means 2 tabs every 4 hours, or 3 tabs every 6 hours, or 4 tabs every 8 hours. Don't exceed 4 tabs at a time. I would start with the higher dosage (4 tabs every 8 hours), and take it around the clock, whether it helps or not. It may take several doses to have an effect.
The second thing I would do is to call the doctor and ask for a prescription for a muscle relaxing drug such as Flexeril. That drug can cause severe drowsiness, but it doesn't sound like she's going anywhere anyway. Taking one of those before bed can really help with the muscle spasms and help her get a good night's sleep.
The other thing she will want to do, and this sounds wrong, but it is necessary, is to get up and move as much as possible. Lying around makes back pain worse. Gentle movement will actually help her get better faster.
A heating pad or hot bath may also feel good.
She will want to stay away from narcotic pain meds - they aren't great for muscular pain and have a load of side effects (constipation, urinary retention, nausea, etc.)

Buy the ibuprofen as a gel you can rub on her back - it'll be better for her than the tablets
Buy some sort of combination acetaminophen (ie. paracetamol) and codeine
Rest her back
Warm pack
Maybe some zinc tablets
She may need to use a wheeled trolley or a chair or something to help support her to get from the bed to the toilet
If the pain is too much call an ambulance and get them to take her to the emergency department - they can give her stronger pain relief there

No surprise to me, I get these types of things everyday! When conventional medicine fails then what??
Try Arnica cream. It is homeopathic and works well when you massage it into the area every 2 hours or so.
If the intial 72 hour period has passed, then cease with the ice and allow the natural heat to circulate again. Be careful when massaging in the crea not to be in any way severe with the pressure. raise the feet slightly and support under the knees.
