The medical term is Sex Reassignment Surgery and it covers both males and females who want to change into males or females. The sex neutral term causes less confusion. When discussing the actual surgery, and the individual, then the sexes involved are clearer.

Its the same when a man gets a sex change: "Gender re-assignment". They are making their outer body match their mental/inner body so that they can finaly feel more complete.
