yes it can.

if your SIM gets damaged, it will give you an "invalid SIM" message. if you enter the pin code wrong thrice and the PUK code wrong for ten times, your SIM gets invalid.

once you have an invalid SIM, you need to replace it with a new SIM. the customer care of your mobile subscription can help you with that.

yes it can go invalid when your details given ( Proofs) are found wrong at any point of time.
When you give worng security code when asked for then SIM cards get blocked
