As others have said, the absolute best way is to read. Read a variety of material - newspapers, magazines, fiction, and non-fiction.
Write a lot and frequently and use a dictionary and thesaurus when you write. Keep a daily journal. Send lots of e-mails and be very strict with yourself about using good English.
Do crossword puzzles - buy crossword books at a level appropraite for you if the newspaper puzzles are too difficult. Play various word games - do a search for word games - there are many at all levels.
Practice writing paragraphs and essays about things that interest you and ask someone to edit them and make suggestions about your vocabulary.
Sign up at a couple of the sites that will send you a new word everyday in your e-mail. Go to the Reader's Digest website and sign up for their word power activity.

Read everything. Not just what you "like" and "know" but all styles of book and other forms of literature. That's how I learned my current new favorite word, "susurrous". Most of the time I've picked up new words from reading books that the definition is clear through the context of the sentence but whenever I hit upon one I'm not 100% sure on I get out the trusty dictionary.

Read as much as you can, play word games, and do word puzzles. Also, if you see or hear a word you don't know, look it up in the dictionary and then try to incorporate that word into one of your own conversations.

read the SAT words from a list that you can get from the internet, just go over 5 words per week or maybe more if you can, go over those words everyday in your mind and by the time you'll have a higher vocab.
