I used to process teacher evaluations, but I don't have any samples for you. They include questions about how well the teacher communicates the information being taught, how they interact with students, how useful the class is compared with similar classes. They usually have space for students to write in comments about what they liked or disliked about the class. Most teachers take evaluations pretty seriously and often use student suggestions in later semesters.

I have like 5 binders of that crap. It depends on what type of evaluation. What state are you in? It is usually based on the Teacher standards. Make sure your lesson plan covers all those standards. We have 9 here.
