Just by the definition of what an enema is - the injection of fluid into the rectum to help relieve constipation - then yes, it would definitely involve inserting it into your anus.
Will the crud just come running out? It would depend on how strong your muscles are at being able to hold it in. But just like what goes up must come down. . . . what goes in, must come out. . . . eventually. That's the whole idea of taking an enema.
As for how to take it? Read the directions on the box, they are very good and meant to be followed.

Most improtant read the directions on the package! It wouldn't be an enema if you didn't insert it in your anus. No it doesn't come running right back out. The directions will tell you how long to hold it before going to the bathroom.

There are many ways.
Lie down on the bed on one side with legs bent so that knees touches chest. Lubercate the nozzle of enema and slowly insert in the rectum and sqeeze thee bottle so that it is emptied. Keep lying in the bed for some time. After few minutes go to toilet and evacuate.

You insert it in your anus, wait a few minutes until you have the urge to go, then you sit on the toilet and let loose. Try not to stress about it
