The Premiership begins the process of deciding on fixtures as soon as the play-off winners are known. it is a long and complicated process that is affected by many factors.
The general principle is that teams should play alternately home and away. Second to this, no team should play more than two consecutive away games.
Consultation takes place with local police authorities to ensure that the police force is not overstretched - for example, the number of neighbouring London teams playing at home is limited so that the Metropolitan Police do not have to police a number of high attendance fixtures at the same time.
Another aspect of police guidance is to advise so as to prevent 'flashpoint' games being played at particular times. For example, although both Norwich and Ipswich fans have a good reputation generally; there has been a history of trouble when they play each other. In recent years because of this the game has often been scheduled for Sunday at Midday - this limits troublemakers spending time in pubs before the game.
Thirdly, a number of other factors are considered. Some clubs have shared grounds with others in the past (for example Crystal Palace and Wimbledon) so when one team is at home the other needs to be playing away) and others share with other sports like Rugby League.
There are a number of 'international' dates in the football calendar when league action across Europe is suspended so that Internationals can be played.
Finally, the teams playing in European Club competitions are given some consideration and often have their Saturday games moved to Sunday or Monday evening if, for example, there is a likelihood that they will have played a European tie on Wednesday or Thursday.
SKY and other broadcasters though have significant influence and fixtures are moved, but the basic structure is decided in advance. Mr Murdoch does not write the fixture list - yet.
In the past there was a tradition of teams playing home and away over the Christmas holiday period - it was common in the 1950s to play a team home and away on Chritmas Day or Boxing Day and New Years Day. This meant footballers saw very little of their families over Christmas, and has become a thing of the past.

Sometimes a club may request an away game for the 1st couple of games if they are having building work done at their stadium and I believe that newly promoted clubs can request a glamourous opening game if they wish.
Unfortunately it can throw out sets of fixtures that can lead to a lot of travel in a short space of time over the holiday period e.g. Charlton at Newcastle d Torquay at Stockport last season.
Also the fact that Man City's final home game this season is against Man United means that there could well be potential crowd trouble at this game if there are titles/relgation at stake.
Utlimately the final say will go to Sky as they re-arrange games to suit the fan in the pub or the armchair and not the paying fan who goes to the watch the game at the stadium

A Computer generates them at randon but certain criteria is added to take in to account teams playing in European competitions and things like Liverpool and Everton or Man City and Man United not playing at home on the same day etc.
