Last I knew, USA citizens visiting the Dominican Republic do need a passport or their birth certificate, but do not need a visa. When entering and leaving the country, a tourist card is needed that costs $10. I once forgot to get that form on the way out and missed my plane and had to borrow some money to do it and get on the next plane.

Granted I was there over 5 years ago, but I didn't need one. I was staying in the wilderness there though and going to Haiti and if you are maybe going to be hiking or anything you might have to get vaccinations and take Malaria pills (I had to). Just ask your doctor about it, and DON'T drink the water ANYWHERE! Seriously!

You don't need a visa, but you do have to pay 'entry tax' (about $10 ) and get your passport stamped.
You'll also get hit for another $10 'exit tax' when your leaving.

yes - its best to apply before you go so that you don't have to queue when you arrive. Address can usually be found in travel brochure.
