Talk to your friend one on one. Inform her that you are not trying to be "motherly-like" or put your nose where it doesn't belong but that you love her and are looking out for her.
Tell her that smoking isn't cool, it can cause wrinkles, liver spots, trigger acne, and will lead to cancer in the future as well.
Also tell her that her body is a temple. It is priceless and she shouldn't let any man take advantage. Let her know that many older men only want sex and that she should stop having sex with such older people and in general until she is more matured. Explain to her that there are a lot of diseases out there these days and people that you have sex with that may be infected wont tell you, so you end up having the disease also.
Tell your friend that you care about what happens to her and that you only want to see good things happen in her future. Inform her that if she keeps acting the way she is she is going to become a whole different person; someone who isn't real.
Persuade her into believing that what she is doing is wrong. Tell her if she ever needs anyone to talk to that she has you and that she doesn't have to go off smoking or having sex if she is unhappy about things (because a lot of teens with family issues get into sex, smoking, and drugs) going on in her life. Assure her that you will always be here for her no matter what but that you want whats best from her; coming from a best friend status she should see the light.
IF however she doesn't see the light after you try hard to help her, let her make her mistakes on her own, eventually she'll learn from them, hopefully she listens to you because if not she could end up with AIDS, HIV, SYPHILIS, CHLAMYDIA, and more.

How old is your best friend? Assuming she's much younger than the 20 year old, ask her this -
If you could do anything you want with a million dollars, what would it be?
The ask her, is this 20 year old in line with what she wants?
Young girls who smoke? Jus makes them look skanky. She likes the skank look?

Eh, when my friends started getting all wild, that's when I broke off the friendship. They were more interested in guys/sex anyway. They're just bad influences and they're not going to stop no matter what you say. They don't care.

Find a way to tell her parents without her finding out it was you. They'll take care of it. Make sure you do something though, because if she's underage, it's definitely illegal.

At the very least please encourage her to protect herself from pregnancy and STDs. Go with her to a Planned Parenthood so she at least won't end up being a single teenage mother with AIDS (imagining worst-case scenario here).
