Who came up with the idea of a first middle and last name.
Also where did it originate from.

Note that this is very culture specific. In some countries, what Americans consider "last name" actually comes first and the "first name" is referred to as the given name (as opposed to "family name"). Examples of such cultures are Japan and Hungary, if I am not mistaken. Also, in some cultures it's much less common to have a middle name than in the US. But in some countries people have even longer names than in the US.

Despite their relatively long existence in the Western world, the phrase "middle name" was not recorded until 1835 in "Harvardiana", a periodical of the time.
