my boyfrien suffers from extremely ichy balls,he has been checked for stds and all negative,any ideas ?its gone on for about 4 years

Jack's got a pretty good starting idea.
Your BF is probably at a point now that it's partly habitual in combo with a laundry soap that may irritate his skin. Maybe he just needs some fresh are more often.

He may want to try changing to a different type of underwear. It sounds like an irritation of some sort. Changing soap may be a solution.
