They don't exactly come untied but they do grow back together and this makes pregnancy possible. The failure rate for a tubal ligation is about 2% and typcially it fails within the first year or so after the procedure. If you do get pregnant after having a tubal ligation you are at an increased risk of an ectopic pregnancy.

They can.I think a tubiligation is only guaranteed for five to seven years.After that they could come untied(which is no very common,but it does happen) resulting in a tubal or ectopic pregnancy.
