Back in the day I was apprentice mechanic so I am well familiar with brake work 20 yrs ago. I am sure not much has change in the way of a brake -drum assembly. Problem is I cannot see the adjuster through the ispection slot I do however see what appears to be a spring about 3/8" dia..

nothing has changed it comes off the same the spiing you see is the adjuster spring the adjuster will be behind the spring just push it up out of the way and you will see the adjuster

the drum uses a nut to hold it on, remove the dust cover and you will see it, just like the front brakes on old rear wheel drive car, and the spring you see is the lower retaining spring and the adjuster is just below it, kind of a pain to get to but it is there.

I had one of those vans. On one of the lugs there is a small clip holding the drum on, it is probably rusted so you can't see it, it is a tight fit. If you got that off, smack it with a hammer a few times to loosen it up some.
