There used to be a "time capsule" in stores, it's the size of a big popcorn tin, comes with a baby book for the parents to fill out. The idea is to fill it with info and things from the day/year of the birth and seal it for the child to open on 18th birthday, or when they have their first child. Everyone I ever bought one for LOVED it.

I love giving unusual gifts myself.

Last time I gave a baby gift, I found a diaper pail, Added in some wipes, electrical outlet plugs, cabinet locks, a basket that fit into the dishwaher for bottle nipples and rings, a nice baby medical kit, with baby nail clippers, a nose sucker, a thermometer, etc.
I included some mylacon drops and infant tylenol, rash ointment, night light etc. All very useful but often overlooked items.

Take your time, look around, I heard about a medicine dispenser pacifier and a thermometer pacifier, these might also be useful to add into the mix.
If you do get her some pacifiers, don't forget the holders, they clip onto babys clothes so if it falls out it won't get lost.

The gift I always get for baby shower's is a diaper bag (one can never have too many) full with the diaper bag accessories. A cute lil baby outfit (onesies here) Then to go with that I buy the johnson and johnson gift bath basket. But I hope this helps :D

I am a mom of almost three (due with 3rd in Dec.) and my favorite little gift was a Hallmark 'Mom-to-be" Christmas ornament that had the year on it (1996 & one in 2003). All of the 2006 Hallmark ornaments are out (I just bought ours today). I love hanging them on the tree every year and remembering that pregnancy. It will be neat to have them when I am 80 and still decorating the tree;-) Just a thought!

Make a cake from diapers. Add little baby things to it for decorations. I saw one before and it was beautiful. Also able to use once the baby is born. It is very creative and shows you put time into it.

I would congratulate her and hug her (or however you express love in your family), and then don't do anything baby related until further along. Alot of women lose that first pregnancy, and then on top of that grief, everyone else is uncomfortable too. Growing a baby is a process, allow it to follow its course and when she's alittle further along give her something she really needs like a massage or spa treatment. Throw her a great shower about a month before the baby is born.

Get her one of those baby sling things, and make some badges for her to put on it. Or decorate it yourself with some fabric paint. If you know the sex of the baby when you do that, try some girl-or-boy designs. Otherwise it's best to stick with neutral designs.

Another thing I saw today that I think is great is colored cloth nappies (diapers if you're american). They were in bright, outrageous colors like fluro pink and red and electric blue, emerald green, etc... I looked at them and it struck me as such a simple but interesting and fun thing to get. Too bad we're going to use disposable nappies when we get our baby.

Although it's a wonderful idea to get this for her, why not wait until the baby shower? You need to know what she is going to need. That's your best bet. But here is an idea.... why not this book...

a jar of pickles and some vanilla ice cream...odd but will come in handy...trust me im 31 weeks and thats my favorite...you could also try to get her a stress ball...they work wonders...congratulations on your niece or nephew goood luck!!!!!!!!! and just to comment on the comment about ALOT of women losing their pregnancy...i wouldnt even consider that an issue unless there was a reason to worry some people dont have a very optomistic look on life...sooo you can buy her or do whatever you want and be happy not worried about oh what if i buy this and she loses it....some people just need to be a lil more considerate...
