A person has been convicted of a crime heinous enough to require the passing of the death sentence. But he is still human. Therefore a sterilized needle is used to acknowledge our own humanity.

I believe that if someone who is in line for lethal injection is deemed ill or unfit, they cannot give them the injection.

It is a matter of legality that there was no possible way that this person was a victim of malpractice.

Say you got an infection like HIV or AIDS from the needle, well then the coroner would get confused and have to put down died from HIV or AIDS related. This way they know to put down Died from Lethal Injection on the Death Certificate. Then again if you did not have the death penalty you wouldn't have this problem then would ya.
