The current tests are fool proof, but if a less specific test is taken (impossible on hospital conditions) some amphetamins or over the counter stimulants including cough medications based on pseudo-efedrin can be mistaken for illegal stimulants.

It can be put in your food without you being aware of it.
It's slightly bitter but in something sweet you wouldn't know it.

If you breath the fumes from someone smoking crack.

Almost any chemical can be adsorbed thru the skin. But you'd pretty much have to get enough on you that you would know about it.

Someone could have laced some weed with the stuff, if you smoke pot.

No, cocaine is a man made product, the only way to have sh it like that in your system is if you are a user or (unlikely) someone slips it to you. It is more likely to eat poppy seeds and have traces of this in your system and have your doc. think you have been using heroin.
