The list of pitchers who've achieved this feat has been pretty well covered by the other respondents. I'll just add a couple items that always intrigue me. Harvey Haddix, in 1959, pictched what has been referred to as the greatest pitched game ever. Haddix pitched 12 perfect innings before losing in the 13th. Also, in 1917, Ernie Shore of the Boston RedSox pitched what remains the only no hitter or perfect game that wasn't when he was brought in as an emergency reliever after the starting pitcher, some guy named George Herman Ruth, was ejected after arguing with the umpire following a walk to the leadoff man. The runner was either picked off or caught stealing, accounts have it both ways. Shore then retired the next 26 batters. To my knowledge neither of these feats is recognized by the current standards as a perfect game or no hitter.

Here is a list of all the pitchers who have thrown an "Official Perfect Game" by definition of a perfect game being "A no-hitter in which no opposing player reaches first base, either by a base hit, base on balls, hit batter, or fielding error; i.e., the pitcher or pitchers retire all twenty-seven (27) opposing batters in order."

Cy Young
Addie Joss
Charlie Robertson
Don Larsen
Catfish Hunter
Len Barker
Mike Witt
Kenny Rogers
David Wells
David Cone
Lee Richmond
John Ward
Jim Bunning
Sandy Koufax
Tom Browning
Dennis Martinez
Randy Johnson

Sure, not a lot of pitchers...but a handful (17 pitchers).

I actually watched the David Cone perfect game on television in 1999.

I also saw Pedro Martinez throw 9 innings of perfect baseball against the Padres in 1995, but the Expos couldn't score for him...so the game went to extra innings where he gave up a double in the 10th to Bip Roberts. So that doesn't count, though some argue that it should.

I think it's been done a shade under 20 times in MLB history.

Off the top of my head I know that Cy Young, Don Larsen (during the World Series, no less), Sandy Koufax, Jim Bunning, Tom Browning (go Reds!), and David Cone have done it.

Plenty. I think Nolan Ryan threw one (maybe more). David Cone and David Wells pitched perfect games within a year of each other.
