well the txt mssg is sent to the phone commpany with a code on it then from the code it is sent to the phone who u wanted it to send to.

All messengers have their own setup protocol.

I can tell you standard way of doing it is to use SIP packets to set up the call. Then RTP packets that carry the voice data.
