To get the secret ending, on Normal mode, you have to complete every sidequest in Jiminy's Journal. On Proud Mode, you have to lock the door to all worlds. You can't get the ending on Begging mode.

Here's the secret ending:
In a dusty barren landscape, save a few plants, the Ansem Reports fly
overhead. There seems to be destruction everywhere. An armored figure with a
cape stands amidst the devastation. He holds a Keyblade over his shoulder. Two
more people in armor approach. They walk to a middleground where Sora, Mickey,
and Riku's Keyblades are stuck in the ground. The destruction behind them melts
away. The three each take a Keyblade. Surrounding them on all sides, creating a
four-way path, is an uncountable number of Keyblades. Through the dust, they
see another figure walking toward them. Words appear on the screen:

"Master of keyblade"

"The lost two"

"Memory of Xehanort"

"Chasers"

"Keyblade War"
