They need to seek cognitive therapy. It will help with their phobia by using a progressive stage of therapy and exposure to whatever they are afraid of.

So you would have sessions with the therapist and then slowly, show pictures of a toilet clean, then pictures of a dirty toilet, then perhaps stand in a clean toilet, and progressively stand in a dirty restroom....

it can work with any phobia.

show them to a toilet thats not cracked at the sides and overflowing with defecation.
