It was never revealed how Phibes knew Vulnavia. They had a rapport that was more implied and understood than explicit. She was in Dr. Phibes Rises Again and never spoke in that one as well. (I think it was a different actress) I don't think she is a mute though because she accomplishes some tasks that required her to interact with others. It was never mentioned that she couldn't speak- or the police would have received word of it during their investigation. She sure did know what Phibes wanted though and that was to bring Victoria back to life.
Glad to know others have seen the movie though- it is very macabre.

All we know is that the girl, whose name is Vulnavia, is mute and Dr. Phibes' assistant.

It was implied that she was some kind of alien. Hence her entrance from the mirrored hallway in the first movie. She just appeared out of the glass. I heard a rumor she is from the stars?

If she was a mute then why in the second movie when the acid poured on her you could hear her scream?

the relationship is never fully explained, other than vulnavia is his assistant, and doesn't speak because she is a mute...
