Does anyone know a web site where I can find how to word a document for temporary power of attorney for my child? She is going away w/ her grandma and I want to make sure I have the right papers if she needs medical attention while she is with her. Thank you.

You can download the necessary form for a small sum with guidance notes. Waste of money to hire a lawyer or getting it notarised, filling out the form will be sufficient. AzOasis8 wording is not sufficient and the signature will need to be witnessed.

It's pretty basic just say I (insert name or change I to we and insert your name and your spouse's name) the parent of (insert child's name) do herby give power of attorney to (insert grandma's name) from (insert start date) to (insert end date). All decisions regarding her medical, physical and emotional well being are to be handled by (insert grandma's name) from (insert start date) to (insert end date).

Then just sign it and have it notorized...

POWER OF ATTORNEY IS NOT NECESSARY. . . .CALL HER DOCTOR OR A LOCAL HOSPITAL . . . THEY USUALLY HAVE "MEDICAL EMERGENCY" FORMS AVAILABLE. . . SIGN IT. . AND HAVE IT NOTORIZED. . .
