If these are supposed to be newspaper headings they should read:
Whole family takes poison! Leaves long suicide letter!
But if you are just speaking of an incident in the past then it would be better to use the past tense:
The whole family took poison! They left a long suicide letter.

I don't think either one of them are right. I would word it..."Whole family consumed poison and left a long suicide note. Is this for a headline or something??? Sounds like it. It makes a big diffrence if your writing a headline or an actual sentence.

Sentence 2 is the more correctly formed of the two.
However, either sentence would suffice for a newspaper headline, since newspapers care less about news than about how fast they can print it.
