emphysema is a disease almost exclusively casued by smoking (it CAN be caused by exposer to carbon, like coal miners, or in congestive heart failure, but 99% of all cases are smoking related)
Through a variety of mechanisms, smoking destroys the elastic recoil tissue of the lungs, disabling their ability to deflate and push air out. As subsequent breaths are drawn in, the alveoli expand and expand, unable to deflate well enough, until they burst. In consequence, this means the total surface area in your lungs for absorbing oxygen is exponentially cut.
It is a progressive, and incurable disease. There is nothing you can do to regenerate the old lung tissue, nothing can be done to recover to how you were pre-disease. The more you smoke once you have it, the worse and worse it gets. So stop smoking immediately, otherwise it's a miserable way to die.
