I work in a hospital in the Emergency room, simply call or go to the facility where you were treated and sign a "release for medical records" form, that is done at the registration dept. or the business office.

I would imagine that under Data Protection you are entitled to see ANY information being held about you and as such you should put a request into your doctor for this information

You have the legal right to see them but your doctor may keep some back on the grounds that they are "sensitive"

I'm not sure you can get a copy of your entire record. I got a copy of one hospital admission and if they hadn't have been at fault for damaging my kidney it would have cost me £42. I will be interested if you can get a full copy.....should make interesting reading...lol xx
