This is all guesing.
Luk was in contact with Obi-Wan through force and Darth Vader is strong in seeing through force. So he could get it that way.
And also how many Skywalkers are on Tatooin conected with Obi-Wan. He saw Luk at the end of Star Wars, felt strong force in the battle of Death Star. How long he would need to figure out that maybe child have survived even if Padme is death.

Episode 5, two ships pass each other, the force comes out to Darth, he says something as such: "I have a fealing I haven't felt since.." then walks off. They meet, he chops hand, he knows.

well i would think that darth vader would know that his real name just happens to end with Skywalker... a bit self explanitory, then there is the whold force powers thing and ya obiwan was with Luke Skywalker...need i go on?
