Dog's mouths aren't necessarily cleaner, they just have different bacteria than our mouths do. She might have to worry about a yeast infection, I don't know if she can get worse from it.
I think it's a little gross. It's called bestiality and can be percieved as animal cruelty by some people.

not only is it very unhealthy (as a dogs tongue has many
types of bacterial) it sounds like your friend needs to talk to a professional this is a pyschological issue as well
