Personally, I don't think you ever "really" know. Part is gut and part are actual "symptoms". Problems occur when a child knows the system and the "symptoms" it's not hard for anyone to manipulate others in to believing what isn't true. I suggest a professional assessment from a psychologist. They can usually accurately determine if a child was abused or making it up.

If a child is afraid of going to school and cries is a bad sign....If a child tightens up or becomes abnormally quiet during the conversation about the orphanage or school...something may be wrong....If a child does not bring home his drawings or awards....something may be wrong....
Don't allow any unusual behavior to go unnoticed....

children are very smart and they certainly no how to manipulate! But if you do suspect a child has been abused i would get advise from someone on a professional level
