In most cases, you will need to submit your old document, change of name document (or marriage certificate if that is the reason for last name change). Financial institutions require an affidavit stating that the person has changed his name. And in an extreme case, you will need to submit copies of the newspaper ads placed to inform your name change. It is best to list all official and bank documents and then take the time out to get your name updated on them at a single shot.

Change of name will not matched with the previous documents. The remedy for you is to make an affidavit that the person in your previous documents is the same as the person with the new name. Have the affidavit notarized and submit it to your school or company for correction of your names.
