Remove your simcard and power up the phone without it. When the screen reads "insert simcard" enter the following code:
If you have Cingular account enter #pw+355716570214003+7#
If you have a T-Mobile account enter #pw+705940422266733+7#
To get (p) press the * key 3 times
To get (w) press the * key 4 times
To get (+) press the * key 2 times
If you entered it correctly your phone will read Code Restriction OFF
If you get an error message try the other code. Please note that you only have 5 attempts, after that your phone will be locked and will not be able to unlock without a data cable and software.

Either you are using a disallowed sim card in a network restricted handset (networks stop you using another network sim in their handsets to protect their subsidy investment) or the phone has been reported stolen, in which case the network will have placed an imei bar on it to stop any sim working with it. Best bet is to call the network operator the phone originated from (ie - which brand is on the front?)

do you have a tmobile sim card in it? if not that would be why. If you bought it as a prepaid phone and tried to put your sim card in it that is linked to an actual plan that could also be another reason.
