custody determines who is responsible for the child ie what education the child will receive, who the family doctor is, even hair colour and types of sports the child will be in.
there are 2 types of custody
1) sole - one parent is the decision maker. this type of custody is generally awarded in cases where one parent is so irresponsible that s/he is not capable of caring properly for a child, and in cases where the parents cannot communicate - such as cases where one parent has been abusive to the other.
2) shared/joint custody - both parents jointly make decisions regarding the child's care. this type of custody is generally decided out of court when the parties can get along reasonably well. most cases which end up in court end up with one parent getting sole custody.
visitation determines how often the parent who does not live with the child gets to see the child.

Yes, there has to be a custodial parent or what in my state is called primary residency. This just means that this is the place the child will establish his legal residency at. Like for school, state aid etc. It has nothing to do with custody however. the divorce settlement will establish visitations, child support etc. You can have joint custody but still one parent will have to be the primary place of residence for the child.

the custodial parent is the parent that maintains a home for the child and the child lives with that parent most of the year, you make all primary decisions for child, both parents can have joint legal custody ,non-custodial parent gets visitation better to both share child and agree to visits or court will decide and neither will be happy with result.

If there is a divorce, one parent have to take custody of their children if the kid is under 18 years of age.

The custodial parent is the one that the children live with and the one that makes most of the everyday decisions about the kids.
I am the Sole Managing Conservator of grandchild. She lives with me and I make all the decisions about her every day care.
Her parents have Possessory Conservator of her and they get to have visitation and pay child support.

You can have shared custody but if one parent has health insurance, it has to be provided to the child by that parent till a cerain age. My kid is to be supported till he graduated HS even if he is over 18 at the time.
