in the film, pet semetary 2, at the beginning, where the kids mother is fried whilst filming a movie. i was wondering if anyone knows if this has ever actually happened on a movie set.

Don't know about that, but actors, stuntmen and crew have been killed making movies. Two children and actor Vic Morrow were killed by a crashing helicopter during the filming of the Twilight Zone movie. There was a big court case because of it.
Amazon lists six movies with "death during filming" including Gladiator.

I have never seen the film of which you speak, but I do know somethings about film making.
I can almost assure you that this did NOT happen in real life. Movie directors (or special effects advisors) are not aloud to harm an actor/actress in real life. Most occurances of actual injury are purely accidental. It's all in the actor's contract.
