Tom Riddle, obviously...
However, I'm not sure the map would give his accurate position. If you remember correctly, when the twin brothers gave the map to harry, they watched dumbledore "pace his study", and one of the twins mentioned that "he did that a lot". However, this seems weird, when you think about what dumbledore says in the 1st book: "I don't need a cloak to become invisible". When put together, those 2 pieces of information tell me that Dumbledore can go into the school, invisble and that he would leave a confundus charm in his study pacing for him while he walks the school grounds unoticed to listen to students and learn what happens in his school.
after all, people aways say dumbledores knows a lot about almost everything. And I would be surprised if he could have learned all of that while staying pacing in his study. OK, the former headmasters could bring some news of the world, but there are limits to that.
And if dumbledore can do somthing like that, voldemort can too. Therefore, do't trust the map when it shows tom riddle somewhereon the map. Tom might be right behind you, at that moment.

His name would appear as: Tom Marvolo Riddle. The map never lies. Lord Voldemort is not his true name.
The map didn't show Peter Pettigrew as "Wormtail" even though he was known by that name, or "Scabbers" which the Weasleys had called him for more that twelve years. No one referred to him as Peter Pettigrew for over a decade, but that is still his true name.
Dumbledore has always referred to Voldemort as Tom Riddle because even though Voldemort likes to think that he has reinvented himself, Dumbledore likes to remind him that he is and always has been the same kid he rescued from the orphanage: Tom Riddle. Making up a new name doesn't change who he is and the map wouldn't be fooled.

I would say Tom Riddle because he wasn't originally known as Voldemort. I don't know though because after he was more or less reborn in Goblet of Fire he might now be completely Voldemort and not Tom Riddle. I have no idea. I wonder if we will get to find out in the books to come.

My guess is Tom Riddle, for 2 reasons:
1. When Crouch, posing as Mad-eye, was seen on the map, it showed his real name.
2. When Pettigrew, who also went by the names of Scabbers and Wormtail, was seen on the map, the name that showed was Peter Pettigrew.
The map seems to show real given names, and Tom Riddle is Voldemort's real name.

OK I'm gonna say Voldemort. Tom Riddle doesn't exist any more and really Voldemort is more spirit than substance. (Keeping in mind, of course that neither exist outside the pages of a bunch of books.) The ghosts at Hogwort's don't show up on the map, do they?
