There's a percentage of people who do know how those things work. They look at previous designs, and see ways to improve on them. These people are called "engineers." Even if a good engineer is a one-in-a-thousand thing, having 300 million people in the USA means that there will be 300,000 good engineers, right? There are also plenty of the engineering bent in other countries.

The really intelligent humans struggle to design and make the devices and the not so intelligent folk buy and use it. Some times the not so intelligent folks buy this stuff even if they don't need it or know how to use them.
