I am looking for simple instructions to add a guestbook to my website.

Proably the easiest way is to register for a service that provides a guest book for you (there are tecnhical issues for most people to do it themselves). You can use Yahoo!Search, Google, whatever, to find one or simply try out http://www.bravenet.com (they offer guestbooks, message boards, etc.) all for free.

First, check to see if the company that host your web site has a guest book available. Many of them do.
If not, do a Google search and there are lots of free javascript guestbooks available that you can download and install as a page on your website.
